Change Name, Address, Telephone

Students may change their address on Inside UTHSCSA (after logging in, click on "SA Self Service" link). Forms are available for change of address, telephone number, social security number, and name. Changes to address and telephone number also may be made by mail or in person. Name changes must be made in person with proper documentation, such as an official marriage license, official adoption documents, official court order name-change documents, etc.
