Registration Checklist

Do in Advance:

• Log into the UTHSCSA Portal using your assigned User ID and Password; click on SA Self Service, then Personal Portfolio. Check & update your home (San Antonio) address and phone number.
• Check to see if you have holds on your record. If so, contact the department that placed the hold (e.g., student health center, univ. police, library, bursar, etc.).
• Check your class list. Are you enrolled in classes?
• Get Parking Permit.
• Get all required immunizations.
• Mail completed Immunization Record card.
• Get ID Card.
• Clear registration holds
• Send final official transcripts from all colleges and universities attended to:

Bring:

• Checks. You will need several blank personal checks. Credit cards may only be used after financial aid checks have been applied to payment.
• Copy of Immunization Record
• Health Insurance Card
• Driver’s License with proof of insurance
