Required Immunizations

Your Immunization Record card MUST be completed by your physician or health care provider and returned 30 days prior to orientation and registration. If you are accepted later than 30 days prior to orientation, mail the completed Immunization Record as soon as possible AND bring a photocopy with you to orientation. You will NOT be allowed to enroll in classes if your immunizations are not up to date.
