Veteran Benefit Certification

The University of Texas Health Science Center at San Antonio’s Office of Veterans Service and Financial Aid is here to help our veterans and dependents through the certification process. To get certified for Veterans Benefits you should follow the following steps:


To contact the VA with any questions please call: 1-888-442-4551. Be advised this line only accepts calls from 7:00 a.m. –5:00 p.m. CST.  When you hear the voice mail menu, press 1. When you hear the next voice mail menu, press 0. This should connect you to a VA representative or at least in queue to speak to a representative.

To process your paperwork to the VA, we will need the following: (Please note paperwork will be accepted no earlier than one month prior to the first class day.)

  • A copy of your respective VA Form Application. See the list of VA Forms below and select the appropriate VA Form (apply via VONAPP –instructions follow).
  • VA FORM 22-1990 Use this form to apply for educational assistance under the following benefit programs: Post- 9/11 GI Bill chapter 33 of title 38, U.S. Code Montgomery GI Bill (MGIB) chapter 30 of title 38, U.S. Code Montgomery GI Bill - Selected Reserve (MGIB-SR) chapter 1606 of title 10, U.S. Code Reserve Educational Assistance Program (REAP) chapter 1607 of title 10, U.S. Code Post-Vietnam Era Veterans' Educational Assistance Program (VEAP) chapter 32 of title 38, U.S. Code, or section 901 or section 903 of Public Law 96-342.

  • VA FORM 22-1990E Use this form to apply for Transfer of Entitlement (TOE) to basic educational assistance under chapters 30 and 33 of title 38, U.S. Code and chapters 1606 and 1607 of title 10, U.S. Code. Use this form only if you are a dependent of an individual eligible to transfer benefits to his or her dependents. The service member's military branch must have approved the request to transfer benefits. The eligible service member must have designated you by name, the number of months transferred, and the period for which the transfer is effective.

  • VA FORM 22-1995 Use this form when: you're changing schools, you're changing your educational, professional, vocational goal, you left your program due to unsatisfactory attendance, progress, or conduct; and you're now reentering the same program, or you were receiving VA education benefits as a veteran and now wish to receive benefits while on active military duty.

  • VA FORM 22-5490 APPLICATION FOR SURVIVORS’ AND DEPENDENTS’ EDUCATIONAL ASSISTANCE (Under Provisions of Chapter 35, Title 38, U.S.C.) Use this form if the applicant is (1) the spouse or child of a veteran who is permanently and totally disabled as the result of a service-connected disability; (2) the spouse or child of an individual on active duty who has been listed for a total of more than 90 days as missing in action, captured in line of duty by a hostile force, forcibly detained or interned in line of duty by a hostile force, or forcibly detained or interned in line of duty by a foreign government or power, or; (3) the surviving spouse or child of a veteran who died of a service-connected disability or who died while a service-connected disability was rated permanent and total in nature. (4) the spouse or child of an individual on active duty for which the evidence shows that the individual is hospitalized for receiving outpatient medical care services, or treatment; has a total disability permanent in nature incurred or aggravated in the line of duty in the active military, naval, or air service; and the serviceperson is likely to be discharged or released from such service for such disability.

  • VA FORM 22-5495 REQUEST FOR CHANGE OF PROGRAM OR PLACE OF TRAINING SURVIVORS’ AND DEPENDENTS’ EDUCATIONAL ASSISTANCE (Under Provisions of Chapter 35, Title 38, U.S.C.) Use this form if you have received Chapter 35 Education benefits previously, are transferring schools, or changing major.

  • A copy of your DD214 -- DD214 must be a “Member 4” Copy (if student is Veteran) – a copy of your “Member 4” DD214 may be requested from the National Archives eVetRecs Web site – www.archives.gov/veterans/evetrecs.
  • Click this link for a copy of the VA Certification Request Form
  • A copy of Certificate of Eligibility - form is mailed to student by the VA once the application has been processed (mandatory to receive CH 33, Post-9/11 Education Benefits)
  • A copy of your Program Plan
  • NEW REQUIREMENT: An updated copy of your Certificate of Eligibility for CH 33,  Post 9-11 VA Education Benefits MUST be submitted each Aid Year. To obtain a copy contact the VA at 1-888-442-4551.

     

Fill out an application online - (VONAPP) Veteran’s Online Application Web site: Go to www.va.gov.
The link for VONAPP will be displayed under “Quick List.”  To navigate to the VONAPP Web site click on the Veterans Online Application (VONAPP) link. The user will be directed to the VONAPP Web site.
